多普勒超声诊断肺心病36例
莸2结果36中35例附合符合肺心病诊断,诊断符合率为97.2%。35例EPV/'VP均大于072,符合率为100%。心电图异常26例,诊断符合率为72%,X线异常29例,诊断符合率为80%经统计学处理P<0005,有非常显著的差异。附表翳例肺心病超声检查项目及标准滋5讨论时区探查部位肺心病病因甚参但病程进展均经肺动脉高压期(PH期)、右C,qlP_,大期及右心衰竭靓该组病例均属心功能失代偿期,诊断符合率虽高'但治疗上仍有一定困难。若能在PH期确诊,可望早期得到治疗,改善予后因而对PH的检测至为重要,我们认为以下三种指标敏感.多普勒超声易获得:(1)肺动脉瓣活动曲线多种研究表明PH时其曲线的每时相均发生改变,该组病例PV”凹均低平,倒(2,26例CD段呈“w”型,9例呈~”字型。(2)MPA及RPV内径增鬼肺心病肺循环阻力增加,肺靴糯折,.心啼{l新乡医学院学报1嘲年期'卷第2期·201·动脉内流速减慢,淤血致MPA及RPA扩张有人经导管检查证实’≥18m均有阱,该组病例出现RPA内径增宽约占88~/o。(3)MPA流速比值(日/、,P)。肺心病所致PH均由于肺循环阻力升高所致,因此在MPA内大部分取样区域都可探及异常形态的血流频谱。有人观察经导管证实为PH者,其E/、,P)072者占94%。也有人认为PH患者E~/PV均大于0,76。本组∞例E/、,P均大于072,因此PH时瞬时流速在一定程度上反映出MPA内血流动力学变化,但与MPA压力之间却无相关关磊而EPv/、,P随动脉压的升高而增长,可较客观地反映肺循环的流体状态以及射血期不同瞬时流速的相对变化,优于单一流速绝对值的变化有人提出EPv/是诊断PH的可靠定性和定量指标。故认为可疑肺心病患封闭疗法治疗软组织劳损1539例者应早期认真检测以上三种指标,可望使患者在PH期确定诊断a
